| Progression of a pressure sore. Stages 1-4. Stage 1, Inflamed skin. May be hard and warm to the touch. Stage 2, Sore extends into, but not through, skin layers, skin is broken. Stage 3, Skin layers are lost. Necrosis extends into subcutaneous tissue but not into fascia. Stage 4, Necrosis extends through fascia into structures such as bone and muscle. Pressure sores can form on the back of the head, ears, shoulders, elbows, buttocks, hips, thighs, legs, and heels. | |
