This contrast enhanced sagittal (from the side) T1-weighted image demonstrates a peripherally enhancing collection along the dorsal (posterior) aspect of the thecal sac and spinal cord which represents an abscess (red arrows) in the dorsal (posterior) epidural space. These usually occur as a result of a bacteraemia (spread of infection through the bloodstream) and not uncommonly from distant sites of infection such as the urinary tract.
