Dermal Sinus and Tethered Cord (MRI)

| This sagittal (from the side) T2 weighted MRI image shows the typical appearance of a dermal sinus tract,which is a small tract extending from the skin of the back into the underlying tissues. It may terminate in the subcutaneous fat or extend all the way into the thecal sac of the spinal canal,where it can cause multiple episodes of meningitis because of the connection with the skin surface. At the bottom of the spinal canal there is a bright signal lesion which is difficult to see because it is surrounded by normally bright cerebral spinal fluid. This bright lesion is an intradural lipoma and it is tethering the spinal cord lower than normally within the spinal canal. The round bright signal focus on the skin surface of the lower back is a vitamin E capsule (bright on MRI due to its fatty nature); this is used to mark the site of visible abnormality for the interpreting radiologist | |
